Date of Loss

Actual Cash Value of Vehicle

Cost of Repairs

Claim or Policy Number

Mileage on Vehicle

**If the title to the vehicle is not signed by customer, we need a FL. State Form  82995.  This is a Secured Power of Attorney that must be signed by the customer.
